العلاج المعرفي السلوكي للألم المزمن : المبادئ والاستراتيجيات

Résumé: يعتبر الألم المزمن مشكلة من المشكلات الشائعة والمكلفة اقتصاديا على مستوى العالم . فعلى الرغم من التقدم الهائل في فهم العوامل الباثولوجية والتشريحية المرتبطة بالألم يظل كثير من الأفراد يعانون من الألم والعجز، وذلك لأن إدراك الألم والاستجابة له لا يعتمد فقط على العوامل الجسمية ولكن يعتمد على العديد من العوامل مثل العوامل المعرفية والانفعالية والسلوكية والفشل في دمج هذه العوامل سواء في عملية التشخيص أو العلاج لن يؤدي إلي حدوث تغيرات علاجية ناجحة . ويهدف هذا المقال إلي تقديم عرض للنموذج المعرفي السلوكي للألم وتوضيح الدور الذي تلعبه العوامل المعرفية والسلوكية في التأثير علي خبرة الألم ثم بعد ذلك عرض لمكونات ومراحل العلاج المعرفي السلوكي وبعض الدراسات التي دعمت فعالية هذا النموذج في علاج الألم المزمن. Cognitive-Behavior therapy for chronic pain: principle and strategy Abstract Chronic pain is both prevalent and costly. Despite advances in understanding the anatomy and development of potent analgesic agents and advanced in surgery procedures, a significant number of people continue to experience pain and related disability. The perceptions of and response to pain are influenced by cognitive, affective and behavioral factors. Ignoring these factors during diagnosing and treating chronic pain are not helpful. In this article a selective review of research supporting the important contributions of psychological factors is provided, a cognitive-behavioral perspective to understanding pain is presented an integrative treatment and rehabilitation approach based on this perspective is described and some of the evidence supporting the effectiveness of this treatment approach is summarized. The cognitive-behavioral treatment should be viewed as important complements to more traditional pharmacological, physical and surgical interventions.
