La Prise En Charge Thérapeutique De La Polyarthrite Rhumatoide Et La Place De La Biotherapie

Résumé: Objectif. Décrire la prise en charge thérapeutique de la polyarthrite rhumatoïde au niveau du service de médecine interne CHU Tlemcen par rapport à celle rapportée dans la littérature. Méthodes. Une enquête descriptive rétrospective a été menée sur les patients suivis pour PR du 1et janvier 2010 au 31 décembre 2013. Résultats. 36 patients sont inclus, un sex-ratio de 0.24, un âge moyen était chez les femmes et les hommes - respectivement 35 .9±4ans, 61 ± 1 0,3ans avec une durée d'évolution moyenne était respectivement chez les femmes, hommes de 6.7±2.3, de 4.3±2.3ans. 52.8% des cas étaient en forme évoluée, le FR et les AC anti CCP ont été révélé positif dans 84.2% des cas et la forme débutante était dans47.2% des cas, 58.8% des cas ayant un FR positif et les AC anti CCP dans 88.2%. La stratégie thérapeutique des médecins intemistes est basée sur: le stade, l'activité et la sévérité - de la PR et le statut de patient. Un traitement de fond a été instauré dans 94,4 % des cas dont 41.2 % de patients ont été traité par monothérapie type le méthotrexate (57.1%), et 58.8 % ont bénéficié d'une association de plusieurs DMARD. - La corticothérapie restait très prescrite durant les premiers mois de traitements et pendant les poussées. La biothérapie n'a jamais été indiquée initialement mais a été prescrite chez 22.2 % - des patients pour compléter l'efficacité du méthotrexate jugée alors insuffisante pour obtenir un statut d'activité proche de la rémission, cette alternative a été prescrite chez des sujets jeunes avec un âge moyen de 41.4±6.8 ans et en absence d'atteinte cartilagineuse. Discussion. Cette enquête montre un respect global des recommandations concernant la prise en charge de la PR par les internistes au niveau de CHU Tlemcen. Abstract: Objective: To describe the therapeutic care of the rheumatoid polyarthritis on the level of the service of internai medicine at the teaching hospitai of Tlemcen compared to that brought back in the literature. Methods: A retrospective descriptive investigation was conducted into the patients foiiowed for RP from January lst, 2010 to December 3lst, 2013. The therapeutic strategy of the doctors internists is based on: the stage, the activity and the severity ofRP and the state of patient. A radical treatment was founded in 94,4% of the cases of which 4 1.2% of patients were treated by monotherapy inciuding methotrexate (57.1%), and58.8% profited from an association of several disease modifying anti-rheumatic drugs (DMARD5). Corticosteroids remained very prescribed during the first months oftreatments and during pushes. The biotherapy was neyer indicated initially but was prescribed at 22.2% of the patients to suppiement the effectiveness of the methotrexate judged then insufficient to obtain an activity close to the remission; this alternative was prescribed at young subjects with a median age of 41.4±6.8 years and who didn't suffer from cartilaginous disease. Discussion: This investigation shows a total respect of the recommendations conceming the treatment ofRP by internists at the teaching hospitai of Tlemcen.
