Dysfonctionnement D’une Dérivation Ventriculopéritonéale Chez Un Nourrisson : Rapport D’un Cas Ventriculoperitoneal Shunt Dysfunction In An Infant :case Report

Résumé: Résumé : Introduction: Le dysfonctionnement des shunts ventriculo-péritonéales par migration du cathéter distal a été rapportée mais l’extrusion à travers la paroi abdominale est rare. Nous rapportons un cas d’extériorisation de cathéter distal pris en charge avec des suites opératoires favorables. Méthode et outils : Notre cas clinique est un nourrisson âgé de 15mois, aux antécédents de méningite et de dérivation ventriculopéritonéale pour hydrocéphalie sur sténose de l’aqueduc de sylvius et qui revient après 04 mois pour l’apparition d’un syndrome d’hypertension intracrânienne avec une extériorisation du bout du cathéter péritonéal a travers l'anus. Une TDM cérébrale a été effectuée et a montré une dilatation active sur cathéter en place. Un cliché ASP montre le cathéter péritonéal qui est enroulé sur lui-même avec sortie du bout distal à travers l’anus. Résultats et discussion : Le patient a bénéficié d’une révision de valve en urgence avec ablation du cathéter distal et mise en place d’un nouveau cathéter en un seul temps opératoire afin de rétablir l’écoulement normal du liquide céphalo-rachidien (LCR). L’évolution par la suite à été favorable avec régression de la symptomatologie initiale. Conclusion : L’extériorisation de cathéter distal est une complication rare de la dérivation ventriculopéritonéale, elle nécessite un traitement chirurgical en urgence pour lutter contre ses complications graves qui peuvent être mortelles. Abstract Introduction: Dysfunction of ventriculoperitoneal shunts by migration of the distal catheter has been reported, but extrusion through the abdominal wall is rare. We report a case of exteriorization of a distal catheter managed with favorable postoperative outcomes. Method and Tools : We report a case of 15 month-old nursling with a history of meningitis and ventriculoperitoneal shunt for hydrocephalus on aqueduct of Sylvius stenosis and who returns after 04 months for the appearance of intracranial hypertension syndroma with exteriorization of the tip of the peritoneal catheter through the anus. A CT scanner was performed and showed ventricular dilatation with catheter in situ. Abdominal x-rays objectified a rolling of the peritoneal catheter on itself with the distal end exiting through the anus. Results and Discussion : The patient underwent emergency valve revision with removal of the distal catheter and placement of a new catheter to reestablish the normal cerebro-spinal fluid (CSF) through flow. The outcome was satisfactory. Conclusion : Exteriorization of the distal catheter is a rare complication of ventriculoperitoneal shunt, it requires emergency surgical treatment to fight against its serious complications which can be fatal.
